Best Practice Recommended Considerations to Assess Patient's Monitoring Plan- Level 2

Assessing Current Practices:

  1. Do they have a scale at home that they can read?
  2. Do they know their dry weight?
  3. Do they keep a list of medications?
  4. Do they understand how to take their medications and adjust their diuretics?
  5. Do they know their own unique signs and symptoms of fluid overload?
  6. Do they know when and how to call the home health care nurse and their provider?
