Best Practice Recommended Questions to Assess Medication Management- Level 2

Assessing Current Practices:
  1. What medications are you currently taking?
  2. Are you using any herbs, or vitamins to help you stay well?
  3. Do you have a system to help you remember when to take your medications?
  4. When was the last time you forgot to take your medications?
  5. What did you do when you realized you had forgotten your medication?
  6. Do you adjust your diuretics? If yes, how do you adjust them?
  7. Have you had any problems related to your medication use? If yes, what have you done about those problems?
  8. Do you have medications that you only take every now and then?  ie if symptoms arise? An example would be nitroglycerin tablets which are taken only for heart/chest pain.
  9. Where do you store your medications?

  1. Where do you store your medications?
  2. How do you remember to take your medications?
  3. How do you dispose of used syringes?

Assessing Resources, Risk, and Readiness
  1. What is your current plan for obtaining your medications?
  2. On a scale of 1-10 how important do you think it is for you to take your medication(s)?
  3. On a scale of 1-10 how confident are you can remember to take your medications?
