Best Practice Recommended Questions to Assess Patient's Current Coping Abilities- Level 2

Assess Current Practices:

  1. Over the past two weeks have you felt sad or depressed?
  2. Over the past two weeks have you lost interest in or pleasure in activities plus:
  3. Experienced a significant change in your appetite or weight?
  4. Had difficulty falling or staying asleep?
  5. Felt restless or sluggish or fatigued?
  6. Experienced a lack of concentration or felt indecisive?
  7. Experienced feelings of worthlessness or guilt?
  8. Experienced thoughts of death or suicide?

Assessing Physical Status:

  1. Have you experienced a change in symptoms over the last two weeks?
  2. Have you noticed a change in your weight or an increase in swelling over the last two weeks?
  3. Is there anything that has caused an increase in emotional or physical stress over the last two weeks?

Assessing Resources, Risk, and Readiness:

  1. On a scale of 1-10 how important do you think it is for you to obtain assistance in managing your stress?
  2. On a scale of 1-10 how confident are you about making changes to improve your stress?
  3. What could you do differently to help manage your stress?
  4. Do you have someone to assist you when you have increased stress?
