Best Practice Recommended Questions to Assess Patient’s Current Risk Reduction for Acute and Chronic Diabetes Complications- Level 2
Assessing Current Practices: - Do you wear identification that states you have diabetes?
- What do you do when your blood sugar is low?
- For you what would be a low blood sugar?
- How often do you experience low blood sugars?
- When did you last experience a low blood sugar?
- What do you do when your blood sugar is high?
- For you what would be a high blood sugar?
- How often do you experience high blood sugars?
- When did you last experience a high blood sugar?
- Have you been taught to check and care for your feet? If so, please explain how you do that.
- When was your last dilated eye exam?
- When was your last visit to a dentist?
- Have you received vaccine to protect you from pneumonia?
- Have you received your flu vaccine this year?
- Do you have a plan in case of a natural disaster or emergency for your diabetes supplies and medications?
- If you smoke, have you considered stopping?
- Do you currently take an aspirin each day?
Assessing Physical Status: - Do you have pain, numbness or tingling in your feet or hands?
- Do you have chronic constipation and diarrhea?
- Do you have problems with nausea, or bloating?
- For male patients: do you have problems achieving an erection?
- Do you have any places on your skin that are very slow to heal?
- Do you put any lotion or cream on your skin, feet or legs?
- Do you have urinary tract infections more than 2 times a year?
- Do you get dizzy when you first sit or stand?
Assessing Resources, Risk, and Readiness- On a scale of 1-10 how important do you think it is for you: to check and care for your feet every day?
- Repeat above question for;
- To prevent, recognize and manage low blood sugars?
- To prevent, recognize and manage high blood sugars?
- To have a dilated eye exam every year?
- To have your cholesterol checked at least once a year?
- To have your kidney function evaluated every year?
- To see your dentist at least twice a year?
- To receive flu vaccine every year?
- To be prepared for natural disasters or emergencies?
- To take aspirin every day?
- To stop smoking?
- To have your GI, GU, CV function evaluated each year?
