Assessing Resources, Risk, and Readiness
  1. On a scale of 1-10 how important do you think it is for you to solve problems related to your diabetes care and control?
  2. On a scale of 1-10 how confident are you that you can solve problems related to your diabetes care and control?
  3. Do you have someone or some group to assist you with your diabetes care and control?
