Important Considerations
- How has the patient improved on the objectives?
- How has the patient's activities changed since an exercise plan has been implemented?
- Does the patient demonstrate self care knowledge related to activity progression? This should include: Safe parameters for exercise, signs and symptoms of over exertion, ways to conserve energy, additional topics related to physical activity such as sexual activity and return to work.
- A patient's post homecare discharge activity plan.
