Progress needs to be continually updated by assessing measurable outcomes. Use the questions in the evaluation area and "Important Considerations" to assist you in developing measurable goals for your patients.

  • Does the patient verbalize and demonstrate increased knowledge?
  • Is the patient maintaining/improving diabetes control?
  • Has the patient been able to minimize risk factors affecting diabetes control?
