Effective disease management requires that all members of the healthcare team have available to them the most updated information and materials and have convenient access to evidence-based guidelines for care.

It is important for the clinician to have knowledge of all tools and resources for patient self-management support (See Patient Resources) and to have a working knowledge of community resources. Each agency should have in place staff competencies so clinicians can demonstrate knowledge regarding all aspects of diabetes care, especially when treating patients in the home.

Included here are helpful, downloadable, easy to read resources and educational materials for clinicians to use to better understand and implement diabetes patient nutritional plans with patients. Clinicians can download and print these resouces and handouts and provide to patients to help in diabetes education. Or clinicians can direct patients' or their caregivers to this Chronic Conditions Website to download resources on their own.
