Best Practices

An individualized approach to nutrition therapy incorporating the best practices for hypertension and chronic stable angina as well as patient preferences is recommended.

Therapy should be based on the presence of co-morbidities and the assessment of patient and family preferences. Approaching lifestyle change as a family enhances long-term compliance and reduces risk for coming generations.

Other considerations include:

  • Dentition/swallowing ability
  • Food availability
  • Cultural appropriateness
  • Willingness to engage in a diet specific to reduce risk associated with cardiovascular disease (hypertension, dyslipidemia, diabetes) or medication regimen such as diuretics or ace inhibitors
  • Finances
  • Cooking facilities
  • Adequate caloric intake
  • Weight history

Assess (6 documents)
Progress towards goals must be assessed every visit with intervention and goals revised accordingly. Especially important to evaluate are patient's verbalized and demonstrated knowledge, behavior and status.
 
Plan (14 documents)
The plan of care should include measurable patient specific goals that are realistic and obtainable. It needs to include buy-in and understanding on the part of the patient and the caregiver. It needs to consider culture, ethnicity, tradition, individual preferences and economic circumstances.
 
Implement (110 documents)
Implementation is the critical stage when an assessment and a plan are put to work. Ongoing communication among all professionals, the patient and the caregivers involved in the treatment plan is essential to maximize status and stability.
 
Evaluate (3 documents)
Patient's response to interventions and teaching needs to be continually evaluated by assessing measurable outcomes.
 
 


The development of the hypertension and chronic stable angina project was made possible by a grant from the New York State Attorney General on behalf of the Attorneys General of all 50 states, DC and Puerto Rico from litigation settlement funds to benefit the healthcare needs of consumers with high blood pressure and angina.
