Why a Chronic Care Clearinghouse for Home Care?

Nationwide, chronic disease care management programs are being used to decrease escalating health care costs through early-intervention strategies and centralizing care under one primary provider and interdisciplinary teams.
This site provides homecare clinicians the ability to access a single source of information about chronic care management models and best practices designed specifically for homecare patients. Access to this information helps clinicians and homecare professionals to help patients with chronic conditions reach their optimal health and functional ability and advance the homecare quality agenda.

The Visiting Nurse Associations of America (VNAA) is the national association of nonprofit Visiting Nurse Agencies (VNAs) and home healthcare providers who care for and treat approximately 4 million patients annually. Our mission is to support, promote and advance the nation's network of VNAs who provide cost-effective and compassionate home healthcare to some of the nation's most vulnerable individuals, particularly the elderly and individuals with disabilities.
