VNAA wishes to acknowledge the Contributing Agencies listed below for generously sharing their resources, forms, educational material and “best practices” to help create the Chronic Conditions Clearinghouse. The contributing agencies developed and tested the resources on this site in order to improve the quality of care and outcomes for their patients with diabetes. VNAA’s goal in sharing these resources is to empower homecare agencies and clinicians throughout the United States to improve patient outcomes and satisfaction.
