This Clearinghouse provides guidelines, recommendations, ideas and tools to help homecare clinicians work with patients and families on the slow, but progressive journey to learn how to manage their chronic illnesses.This site has been set up to meet the needs of the homecare clinician.
Chronic Care Clearinghouse users can select a chronic condition (diabetes, heart failure, with COPD arriving next summer) and then research and download "Best Practices" in seven key areas of chronic care management:
- Nutrition,
- Exercise,
- Monitoring
- Medications
- Problem Solving
- Coping
- Risk Reduction

At the beginning of each Focus Area you will find Best Practice Guidelines for the homecare setting.

The site is set up according to Nursing Practice Guidelines and each of the seven areas of focus for chronic disease management is broken into:
Assess, Plan, Implement and Evaluate. Under each area you will find questions, resources, tools, recommendations that will provide you with information to use in assisting your patients and their caregivers in managing their chronic illness, reaching measurable outcomes and providing quality care. You may either follow through each section or go directly to your area of interest or need.
