Chronic Care Clearinghouse Series

VNAA Member Benefits

The current VNAA Chronic Care Clearinghouse Series was developed by the Visiting Nurse Associations of America (VNAA) through a partnership with the Home Care Information Network (HCIN), an online homecare education center that provides video-streamed presentations from leading homecare experts.
This partnership enhances VNAA's ability to provide convenient, state of the art educational content to VNAA member agencies and staff and for free. Continuing Nurse Education certificates are offered for every presentation in the CCC Series upon completion of required evaluation forms.

The Presentations

Each of the following presentations will be added to your HCIN account every other week for the next six weeks. By early May 2008, all of the presentations will be available for VNAA members to view by logging into your HCIN accounts. More presentations will be added to the CCC Series throughout 2008, check back frequently!

The current CCC Series consists of four video-streamed seminars on chronic disease management:

Account Information
Every VNAA member agency received their HCIN account log-in information in January, 2008. If you did not receive your account information to log-in and view the presentations, please contact VNAA's Manager of Educational Programs.

Agencies that are not members of VNAA will have the opportunity to access the CCC Series later this spring for a nominal fee. More information will be coming soon.
