The questions and tools listed below will assist in the total evaluation of measurable outcomes for diabetes patients.

Questions To Be Considered:

  • Has patient’s ability to recognize potential problems and seek resources improved if indicated based on initial assessment?

  • Is the patient maintaining or improving diabetes control?

  • Has the patient been able to identify and resolve problems related to diabetes care and control?

  • Does the patient have a solid understanding of blood glucose control?

  • does the patient understand lipid and blood pressure management?

  • Does the patient have a grasp on the importance of high quality foot care and retinal screening?

  • Does the patient show an understanding of the importance of medical nutrition therapy and smoking cessation?
